According to a 2002 interview published in Mojo magazine, former Capitol president Alan W. Livingston stated that it was Paul McCartney who pushed strongly for the photo's inclusion as the album cover, and that McCartney reportedly described it as "our comment on the war".
Apparently at least some copies had the "trunk cover" pasted over, and those are worth a tidy sum to collectors. Greg, according to the above-quoted Wikipedia article, this was indeed a North American release only.
I'm answering my own question! The remastered releases now, as far as I can tell, are the same the world over, based on the original UK masters and track listing. Some of the original Capitol releases were slightly differently mastered, often with more reverb to suit the perceived US market. One day, when I have a turntable again, I'll experiment listening to my original LP, then the 1990s CD and finally the remastered set for a couple of tracks to see if I can pick out any differences!
The pairing of "Tears Dry On Their Own/Wake Up Alone" kills me every time. The heart of the album right there. Unfortunately I think me copy of this from emu is marred by frankenhoffer. I don't know how to tell for sure, but it sounds a little rough in spots, and it's on Universal...
Golly, I've never seen the "butcher" cover (although it sounds very vaguely familiar) - well I was a mere wisp of a child at the time (5th grade?). I'll check the cover for paste over. I think I bought it at Woolworth's.

In the last 24 hours I had a a cancelled flight that made me switch from an early morning flight to one the evening before and add a hotel stay, then that one was hours late so I got to my hotel at 1a.m. with a 5a.m. start to get the next flight this morning, then the next two were delayed as well...

Yes Doofy it was never released here. Presumably with the remastered series they have reverted to the original UK issues of albums and tracks?
